| 内容記述 | 終末期の患者ケアにおける食事の目的と管理栄養士の役割についての見解を、北海道内の49病院に職員として勤務する女性でかつその業務に栄養指導を含んでいる管理栄養士109名と、同じく道内の19病院に勤務する女性の看護師777名との間で調査して比較研究を行った。多変量解析の結果、両者共に食事は重要であるとの認識は一致していたが、終末期における食事の目的と管理栄養士の役割について、管理栄養士と看護師の間で見解に差異が認められた。管理栄養士の方が看護師よりも終末期の患者ケアにおける食事の治療的側面をより肯定的に捉えていることが示された。管理栄養士が現在実施している食事ケアに対する看護師の評価は、管理栄養士自身の自己評価よりも低かった。管理栄養士に今後期待される終末期ケアに関しては、看護師は管理栄養士自身が期待しているほどの高い評価はしなかった。食事ケアは、管理栄養士と看護師とが共に関わるケアである。その意味において、これまでの患者ケアの実践の中で培われてきた看護師の食事と管理栄養士が行なう食事ケアへの意見は、分析に値する貴重なものであるであろう。 | |||||

| 内容記述 | A comparative study was made on points of view of registered dietitians (RDs) and nurses on the goals of eating and the role of RDs in the care of patients at the end of life. The subjects of this study comprised 109 female RDs who participated in the nutritional care of patients in 49 hospitals in Hokkaido and 777 female nurses who worked with inpatients in 19 hospitals in Hokkaido. The result from multivariable analyses showed that RDs were different than nurses in their viewpoints on the goals of eating and the role of RDs in patient care at the end of life, whereas both of them appreciated the importance of eating at the end of life. RDs appreciated a therapeutic aspect of eating at the end of life more positively than nurses did. Nurses appraised nutritional care provided by RDs in hospital wards less highly than RDs did themselves. Nurses estimated a necessary role of RDs in patient care at the end of life more modestly than RDs did themselves. Both RDs and nurses take part in nutrition care of patients in a hospital setting. In this sense, viewpoints of nurses on eating and the role of RDs in the care of patients will be worth analysis and appreciation. | |||||
